Joining Catamount for a ride? A parent or legal guardian must complete this form before their child participates. Please submit one form per child — you only need to complete it once, and it covers future rides as well. If anything you have told us changes, please let us know at carson@catamountracing.org.
Submitting this form does not confirm a place on a ride. Participation must be arranged with Catamount Racing in advance.

Assumption of Risk, Release & Indemnity
Catamount Racing | Guest Child Rider Assumption of Risk, Release, and Indemnity Agreement (GCR-V10.31.26b)
Important: This document is a release of certain claims. Read it carefully before signing.
In consideration of Catamount Racing permitting the Rider to participate as a guest in Team activities, including mountain biking, training rides, skills clinics, travel associated with Team activities, use of Team equipment, and related activities (collectively, the Activities), the undersigned Parent(s), on behalf of themselves and, to the extent permitted by law, the Rider, agree as follows. This Release applies to every Team activity in which the Rider participates as a guest unless and until the Parent(s) withdraw it in writing.
Acknowledgment of risks. The Parent(s) understand that the Activities include inherent and other risks, known and unknown, including falls; collisions with riders, vehicles, fixed objects, wildlife, or obstacles; uneven or changing terrain; rocks, roots, drops, jumps, and trail hazards; weather and temperature; equipment failure; negligent acts or omissions of other participants; remote locations; delayed access to emergency care; serious injury, permanent disability, paralysis, and death. The Parent(s) voluntarily permit the Rider to participate despite these risks.
Release of ordinary-negligence claims. To the fullest extent permitted by California law, the Parent(s), for themselves and on behalf of the Rider, release and discharge Catamount Racing and its current and former directors, officers, employees, coaches, volunteers, agents, sponsors, affiliated entities, and landowners or facilities made available through the Team (collectively, the Released Parties) from claims, demands, causes of action, damages, or liabilities arising from or related to the Rider's participation in the Activities, including claims based on the ordinary negligence of a Released Party.
Limits of release. This release does not apply to claims that cannot lawfully be released, including claims based on a Released Party's gross negligence, recklessness, willful injury, fraud, or other conduct for which release is prohibited by law.
Indemnity. To the fullest extent permitted by law, the Parent(s) will indemnify and hold harmless the Released Parties from third-party claims arising from the Rider's conduct in the Activities or the Parent(s)' breach of this Guest Child Rider Release, except to the extent caused by a Released Party's gross negligence, recklessness, willful injury, fraud, or other conduct that cannot lawfully be released or indemnified.
Medical care and personal property. The Parent(s) authorize emergency care as authorized in the Emergency Medical Authorization in this form, and accept financial responsibility for medical and transportation expenses. The Released Parties are not responsible for loss of or damage to the Rider's personal property, except to the extent caused by conduct for which liability cannot lawfully be released.
Severability and governing law. This Release is governed by California law. If any part is unenforceable, the remaining provisions will remain effective to the fullest extent permitted by law.
By signing below, the undersigned confirms that they have carefully read this Release, understand that it includes a release of ordinary-negligence claims, and sign it voluntarily.
Emergency Medical Authorization
If a parent or guardian cannot reasonably be reached, parents or guardians authorize the Team to obtain emergency evaluation, treatment, and transportation for the Rider. The Team will make reasonable efforts to contact the parent or guardian and the designated emergency contact. Parents or guardians remain financially responsible for resulting medical and transportation expenses.
Parents or guardians confirm that the Rider has health insurance or otherwise accept responsibility for the Rider's medical expenses. Families should review their own coverage for cycling and travel. Unless the Team expressly confirms coverage in writing, families should not assume that participation includes Team-provided health, accident, disability, travel, property, or life insurance.
Parents or guardians authorize the Team to share relevant emergency and medical information with emergency personnel and Team representatives who need it to protect the Rider's health or safety. Families are responsible for providing accurate contact information and ensuring that a parent, guardian, or designated emergency contact can be reached during Team activities.
